As a participant in On-the-Job Training, I realize that inherent dangers exist. While I am in good health and able to participate in such activities fully, I recognize that my participation may result in illness or injury due to accidents, the forces of nature, or other unforeseen causes. Such diseases or injuries may include sickness, pains, strains, sprains, fractures, dislocations, paralysis and/or death. Therefore, I knowingly assume all liabilities and risks resulting from these activities.

On behalf of myself, my family, or other personal representatives, I hereby agree to release, hold harmless and indemnify the COMSOFIL International Inc., BODs, Officers, and the Instructor(s) from any and all claims and/or lawsuits for bodily injury, property damage, wrongful death, loss of services, etc. which may result from my participation in the activities mentioned above, regardless of whether or not these claims or suits arising from negligent acts, omissions by the COMSOFIL International Inc., leaders or facilitators of the activity or any other person involved or from any other cause.
